About the opportunity
We are looking for a motivated team member to join our Active Recovery Team (ART) for the next 12 months covering parental leave. The ART Perth North Pilot is a recovery focused collaboration between North Metropolitan Health Services and Luma, providing coordinated support and treatment for individuals with severe and/or persistent mental health issues, who have multiple treatment and support needs. The pilot focuses on providing recovery focused intensive psychological support to keep them well in the community.
Working closely with the wider team, you will be responsible for providing emotional and practical support, case management, and counselling to clients on an individual and group basis. You will also assist with identifying their needs for wellness, options for recovery, and support to navigate their way through community services and supports.
Key responsibilities include:
- Undertaking clinical assessment of clients and potential clients, some of which may present in crisis.
- Providing advice (and redirection, where required) to clients, their families, carers, and other service providers relating to clients who need support engaging in community services, networks, and programs.
- Facilitating Mental Health and/or AOD support groups, SMART recovery groups, and information sessions to clients accessing the service as required.
- Providing one-to-one counselling, advocacy, and support to participants.
In this role you will need superior skills for showing warmth, empathy and care with clients experiencing complex co-occurring concerns, including; mental health, substance misuse, family domestic violence, developmental trauma. A tertiary qualification, or equivalent experience in a relevant discipline and a sound understanding of recovery orientated practice will also be required.
We will be looking for someone with clinical and problem-solving skills with a demonstrated ability to work alongside people with multiple needs and complex trauma using a case management approach to treatment.
This is a 12-month fixed term, part time position (0.8 FTE or 0.9 FTE) based out of our Wanneroo and Joondalup offices. Travel to other locations will be required on occasion.
