Client Liaison Coordinator - Aged Care

Here at MSWA, we specialise in providing personalised care and support to those living with neurological conditions (for example MS, Parkinson’s, Huntington’s, Motor Neurone, Acquired Brain Injury and Stroke).  We are proud of the range services we provide our clients including Allied health and Clinical Support and Community Services.

MSWA have an exciting opportunity for a Client Liaison Coordinator to join our team and support our Home Care Package clients. This position is based in our Wilson Office.

You will be responsible for the coordination, care planning and monitoring of clients to ensure a quality service is provided.  You will work closely with other team members including our Aged Care Coordinator, Nursing and Allied Health teams.

The successful candidate will use their highly developed skills to help guide our Clients through their Aged Care Journey using a holistic and person centred approach. 

In this role you will:

• Conduct initial and ongoing assessments in the clients homes.
• Development of Care Plans in partnership with your clients.
• Regular monitoring of clients budgets.
• Be solution focused, flexible and adaptable.
• Ensure compliance and legislative requirement are met.
• Respond to client changing needs.

Aside from that, you’ll be making a difference, leading to both positive and successful client outcomes.

Our ‘must haves:’

• Experience or qualifications with Home Care Package Services – ideally within a similar type role.
• Knowledge of Aged Care funding.
• High level of interpersonal and communication skills.
• A genuine passion to make a difference.
• Current Australian “C” Class Drivers Licence with no restrictions.
• NDIS Workers Screening Check (or willingness to obtain.)
• Exceptional time management and organisational skills for prioritising your workload, meeting conflicting deadlines in a busy and demanding environment.
