As a Client Liaison Coordinator, you’ll be joining our Client Engagement team, which is a complementary service we provide to our clients.
Based in our Wilson services centre, you’ll oversee a case load of NDIS Clients. You’ll help connect them to internal services, guide them through their NDIS and help them reach their defined and funded goals!
Every day will present new opportunities and challenges, but they’ll be a few core tasks including:
Building rapport with clients, overseeing their NDIS plan and budget
Conduct assessments with clients to identify their needs and goals.
Provide Services including advocacy, planning, referrals, and yearly review reports.
Develop individualised support plans for clients in coordination with them and their support network.
Identify and coordinate appropriate service providers and supports for clients.
Provide ongoing support and guidance to clients and their support network to ensure that goals are achieved, and services are delivered in accordance with their plan.
Monitor client progress and make necessary adjustments to their support plan as required.
Maintain accurate and up-to-date records of client’s progress and service delivery.
Communicate effectively with participants, their families, service providers, and other stakeholders.
Aside from that, you’ll be making a difference, leading to both positive and successful client outcomes.
Our ‘must haves’
Experience or qualifications in Disability Services– ideally within a similar type role.
Knowledge of NDIS funding.
High level of interpersonal and communication skills.
A genuine passion to make a difference.
NDIS Worker Screening Check (or willingness to obtain).
Current Australian “C” Class Drivers Licence with no restrictions.
Exceptional time management and organisational skills for prioritising your workload, meeting conflicting deadlines in a busy and demanding environment.
