The Role: Intake and Assessment Officer Are you passionate about making a tangible difference in people's lives? As our Intake and Assessment Officer, you'll be the gateway to empowering community members through our suite of support services, aimed at fostering independent living and enhanced community access. This vital position involves the holistic assessment of community members, orchestrating the intake process, and crafting personalized support plans for clients navigating the My Aged Care, NDIS, and Community Transport programs.
Your Impact:
- Serve as the first point of contact for community members seeking support, guiding them with empathy and expertise.
- Manage participant referrals with a holistic approach, ensuring a seamless transition into our programs.
- Elevate administrative efficiency and accountability, setting the standard for excellence.
- Champion the development and expansion of our programs, creating new avenues of support and opportunity.
Your Pathway:
- Report directly to the Service Development Partner, shaping the future of Aged and Disability services.
- Collaborate with a dedicated team, with no direct reports but a significant impact across all organizational programs.
Your Mission:
- Embody and promote Peppercorn's core values and mission, leading by example.
- Uphold our commitment to safety, quality, and continuous improvement, ensuring the highest standards of service and care.
- Engage with our community, building lasting relationships that foster trust and respect.
- Drive continuous improvement and quality management, ensuring our services meet and exceed industry standards.
Who You Are:
- Qualified with Cert IV in Individual Support / Aged Care / Disability or a related field.
- Experienced in community care assessment and support planning, with a knack for engaging those in need.
- A problem-solver with excellent documentation skills, attention to detail, and high-level computer proficiency.
- A holder of a current NSW driver's license, ready to make a difference in the Hawkesbury, Nepean, and Blue Mountains areas.
- Committed to inclusivity, with an understanding of the barriers faced by culturally diverse and minority communities.
