Clinical Care Co-Ordinator

Southern Cross Care (Broken Hill) has been a premier provider of aged care services for over 40 years, with a focus on delivering a high level of individualised personal care, wellness, and enablement for the most vulnerable in our community. 

We are seeking to fill the position of Clinical Care Co-Ordinator.

This position will be responsible for providing clinical leadership, engaging employees to provide our residents with person centred care of the highest quality, whilst empowering continuous improvement and meeting legislative, regulatory and policy requirements.

To be successful you will have:

• Right to work in Australia
• Bachelor of Nursing Degree
• Unrestricted registration with AHPRA
• Post Graduate qualifications and/or relevant experience in aged care will be highly regarded.

Key tasks:

• Monitor and ensure accurate and timely documentation and reports are completed as required by legislation and standards
• Support the ACFI assessment submission and claiming process
• Lead a culture that engages employees through communication, recognition and collaboration
• Actively participate in quality activities that ensure the provision of excellent resident outcomes including compliance and accreditation standards
• Co-ordinate the site clinical and care workforce management, to ensure the site is appropriately resourced to deliver high quality care that is individually focused
• Drive a customer focused approach across all areas of the business through leadership and role modelling
• Ensure processes are in place for work to be carried in a safe manner.
