Care Coordinator – RMH@Home Subacute

This role will collaborate with the RMH@Home subacute team and assist in supporting the care needs of patients admitted under the Pathways to Home Program. This program is set up to help transition people to home from hospital until their NDIS package is in place. The care coordinator will assist to manage any complex discharge concerns and clinical issues whist on the @ Home program.
Your new role:
- is pivotal in assisting the team to Admit, Coordinate and Discharge patients on the service.
- Your role assists in helping to facilitate progression of care and patient flow both through the organisation as well as within the RMH@Home Subacute Service.
- You will be working closely with a team of coordinators who assist with this process.
- You will understand the importance of providing clinical leadership and management which focuses on the best possible outcomes for your patients.
- Work closely with a fabulous and highly experienced multidisciplinary team
- Work collaboratively with consumers, their families, general practitioners and specialists at The Royal Melbourne Hospital
- Support the ongoing development of the RMH@Home subacute service
- Provide advice to RMH @ Home staff to support clinical decision making
- Support the coordination of day to day task allocation and rostering within the team
- You are a Registered Nurse with AHPRA
- You have a sound knowledge of NDIS and preferably have worked with NDIS clients previously
- You have experience working in hospital and/or community settings and understand the factors contributing to hospital admission in clients with complex health issues
- You are flexible, energetic, innovative, and can work cooperatively in a multidisciplinary team as well as autonomously
- Demonstrated organisational skills and ability to prioritise work whilst maintaining a commitment to quality of care and best practice.
