Lismore Community Health is seeking a Clinical Nurse Specialist Grade 2 (CNS2) Chronic Disease Management to coordinate and deliver high quality nursing care to patients/clients in accordance with the Nursing and Midwifery Board of Australia’s National Standards of Practice for Registered Nurses, criteria for CNS2 as outlined in the Public Hospital Nurses and Midwives (State) Award and the Policies and Procedures of the Ministry of Health (MoH) and Northern NSW Local Health District (NNSWLHD).
Providing care coordination, case management and support to people with Chronic Disease, their carers, General Practitioners, practice staff and staff involved in the provision of care. Working with relevant stake holders to ensure coordination of services, utilising shared care plans and other team care approaches to optimally support people with Chronic Disease within the framework of the Chronic Disease Management Service.
About you:
- Current registration with the Nursing and Midwifery Board of Australia as a Registered Nurse (Division 1) and at least 3 years’ full time equivalent clinical experience in the management of clients with Chronic Disease; with a post graduate qualification in a specialty related to Chronic Disease
- Clinical leadership skills and the ability to work collaboratively within a multidisciplinary framework to plan, deliver and coordinate patient care
- Understanding of the NSW Chronic Disease Management Program and an understanding of the principles of Chronic Disease Management
- Knowledge and experience working in Chronic Disease Management in and across the acute, community and primary health setting including client/carer assessment, problem identification and goal setting, client/carer education, individual care plan development and review and care coordination
