About the Position:
Ngaanyatjarra Health Service is seeking a Community Care Coordinator to provide culturally appropriate services to participants of the Home and Community Care Program (HACC) of the Ngaanyatjarra Lands. This position is located in the Kiwirkurra Community and will operate under the direction of the Community Service Coordinator.
The successful candidate will be required to:
- Encourage employment of community members in the Ngaanyatjarra Lands.
- Supervise and assist employees to ensure clients are getting the care they need as directed in their individual support plans.
- Oversee the local meal service and support as required.
- Deliver meals and medication supervision as necessary and in line with the client support plans and direction from the Community Service Coordinator.
- Oversee and assist clients with social activities at the care centre.
- Report to Community Service Coordinator any changes observed in the client or their surroundings that may adversely impact the client or service delivery.
- Maintain all client records and service delivery records daily and maintain client confidentiality at all times.
- Provide and maintain records of services delivered through the systems provided.
- Ensure maintenance of a safe and hygienic working and care environment.

The successful applicant will possess:
- Relevant experience in a similar role working with community members from remote Aboriginal Communities
- Demonstrated cross-cultural understanding of the service delivery of the HACC program.
- Demonstrated commitment to working with participants of the HACC program to achieve desired outcomes.
- Strong motivation and commitment to the principles of the HACC program.
- National Police Clearance or the ability to obtain one.
- Driver's Licence and ability to travel via 4WD on unsealed roads.
