See more at our website – www.kambuhealth.com.au About the RoleThe primary objective of the Community Liaison Officer role is to advance the visibility of Kambu Health services, facilitating enhanced Indigenous access to culturally sensitive primary healthcare services and initiatives. The Community Liaison Officer plays a pivotal role in advancing the objective of narrowing the gap in life expectancy for Aboriginal and Torres Strait Islander communities by eliminating obstacles to accessing suitable healthcare.
The Community Liaison Officer will be responsible for identifying a Patient Engagement Action Plan that involves developing strategies that increase Kambu’s caseload of new Aboriginal and Torres Strait Islander people, as well as reduce the rate of inactive Kambu Health clients. Within this role, you will also be required to: Promote Kambu Health's services and programs within Indigenous communities to improve access to culturally appropriate healthcare. Develop and implement a Patient Engagement Action Plan to increase our caseload of new Aboriginal and Torres Strait Islander clients.
Foster relationships with community members, organisations, and stakeholders to understand healthcare needs and concerns. Collaborate with internal teams to ensure that services and programs meet the needs of Indigenous communities. Participate in community events, meetings, and forums to raise awareness about healthcare issues and resources.
Monitor and evaluate the effectiveness of outreach efforts and adjust strategies as needed. Maintain accurate records and documentation related to community engagement activities and outcomes. Reinforce positive messaging to clients and community members, regarding compliance to health advice provided by qualified health practitioners in addition to self-management of health conditions (specifically chronic disease).
About you Relevant tertiary qualification of any one of the following: Certificate III or Certificate IV Primary Health Care. Previous experience in community engagement, outreach, or a related field is preferred. Knowledge of Indigenous cultures, histories, and healthcare needs.
