The Role:
Council is proud to collaborate with the Australian and Queensland Governments to auspice a newly created part-time position of Community Services Coordinator, a role that has been designed for the purpose of improving access to primary care for our Quilpie Shire residents, and to reduce pressure on our hospital emergency department.
The South West Primary Care Pilot, Care Collective – South West, is part of this initiative, and this locally designed pilot aims to improve integration and enhance hospital and primary health pathways for residents.
The Care Collective workforce aims to reduce the impact that chronic health conditions can have on a person’s life, and to improve the health and wellbeing for those involved by understanding the local areas services, programs and providers, the complexity of complex and chronic health conditions within a holistic approach. A key component for success of the program is clear communication within the hospital and health system, primary care and Community Services, and ensuring timely and appropriate referrals as needed to meet the client’s needs.
The successful applicant will be an experienced program or project officer with a wide range of skills in providing support to a complex environment. The position will work collaboratively in a diverse team environment developing and maintaining effective relationships with multidisciplinary stakeholders.
This is a part-time position for 50% of a full-time equivalent, for the maximum term period to Friday 25 April 2025. The hours of duty and working roster will be negotiated with the successful applicant.
