Key Responsibilities:

- Goal Achievement: Assist clients in achieving their personal and social goals, supporting them in identifying and accessing relevant services and programs.
- Community Activities: Facilitate and accompany clients to community events, activities, and appointments, enhancing their social and community engagement.
- Well-being Support: Offer emotional and practical support, contributing to the overall well-being of clients.
- Advocacy and Networking: Act as an advocate for clients, liaising with other service providers and stakeholders to ensure clients' needs are met and rights are upheld.
- Care Planning: Collaborate with clients and their families to develop personalized care plans that reflect their needs, preferences, and aspirations.
- Feedback and Improvement: Collect feedback from clients and contribute to continuous improvement efforts to enhance service delivery.
Ideal Candidate:
- Compassionate, with a strong commitment to supporting and empowering individuals and families.
- Previous experience in community services or a related field is highly regarded.
- Excellent communication and interpersonal skills, capable of building meaningful connections.
- Ability to work autonomously and as part of a multidisciplinary team.
- Current First Aid Certificate, Working with Children Check, and Police Check (or willingness to obtain).
- Valid driver's license and access to reliable transportation.
- Flexibility to work across various shifts, including weekends and public holidays as needed.
