Better World Care is a registered NDIS provider. We work very closely with our participants to assist them to live their best posible lives on their terms and accordingto their life goals. Our teams provide 24/7 support to NDIS participants across the Rockhampton region through individualized service delivery including in home personal care, community participation and community Nursing care.
You will be responsible for:
- Coordinating the provision of direct services for participants, resolving situations as they arise, and ensuring ongoing communication with staff, participants and their families.
- Stay updated with NDIS legislative requirements and company directives to ensure full compliance.
- Prioritise workplace health and safety, addressing and documenting any emerging requirements.
- Foster a workplace culture in line with our company mission, vision, guiding principles, and values.
- Ensure proper risk assessment is carried out for all participants.
• Assist in coordinating the day-to-day activities, routines and systems which support the daily living and operations of your participants.
• Develop and implementing high quality Support and Care plans.
• Collaborate with participants and relevant stakeholders to gather input for developing person-centered risk assessment, Care/ Support plans and assist participants to work towards achieving their individual goals.
• Ensure all the participants’ case notes, progress notes, incident reports, communication records, recording charts and house files are up to date.
• Undertake all necessary reporting including compliance reporting (reportable incidents, restrictive practices, etc.) in a timely manner.
• Deliver a consistent and quality service to internal and external stakeholders.
• Act as the first point of contact of your participants and staff.
• Assist and support your participants in their medical and NDIS Plan reviews.
• Organise regular audits to ensure high level of quality service delivery. - Collaborate with participant's decision-makers, stakeholders, and internal colleagues like rostering and payroll for streamlined operations
- Actively foster open communication, participate in regular staff meetings, and contribute to policy reviews.
- Ensure timely communication of participant information, especially when time sensitive.
- Collaborate with the leadership team in developing, implementing, and evaluating training and behaviour programs tailored to participants.
Mandatory requirements:
- Demonstrated skills and leadership experience within the NDIS sector. (min 2 years)
- Yellow cards/NDIS worker's screening
- Driver’s License & access to a reliable vehicle
- Relevant tertiary qualifications
