FBA Care Pty Ltd is seeking a values-driven and proactive Community Worker to provide specialist knowledge and expertise in coordinating internal services to support individuals with disabilities to access NDIS. This position will contribute to the Community Connections Program to ensure innovative and client-centred service delivery to meet current and future needs.

Key Responsibilities:
- Implementing NDIS plans.
- Ensuring compliance with all policies.
- Assessing participants’ needs and planning, developing, and implementing support programs.
- Meeting participants to discuss and assess the nature and extent of difficulties.
- Reporting the progress of participants.
- Referring participants to agencies that can provide additional help.
- Assessing needs and resources effectively.
- Supporting and caring for participants.
- Monitoring and evaluating the effectiveness of the support provided, making adjustments as necessary to improve service delivery and worker satisfaction.
Essential Criteria:
- A sound understanding of disability and its effects on social relationships, independent living, daily needs, physical health, vocational, and employment needs of these participants.
- Excellent communication skills, including written and verbal, as well as negotiation and interpersonal skills.
- Ability to identify, organise, and utilise relevant resources effectively.
- Ability to function as a constructive and collaborative member of a cohesive team.
- Demonstrate qualities of empathy, compassion, and integrity.
