About the role:
Reporting to the Aged Respite Operations Manager, a Coordinator actions comprehensive management of care services, including responding to My Aged Care referrals by completing care plans and assessments for support workers under the Senior Coordinator's guidance. It emphasizes the use of Wellness and Re-enablement approaches, regular evaluation of services with clients or carers, and efficient rostering to achieve a 75% billable time target.
Responsibilities include maintaining up-to-date health management and documentation, timely incident monitoring and reporting with client/family support and ensuring accurate consumer file documentation while upholding privacy. Additionally, the role supports program development, data entry, and report preparation for management and government departments, participates in continuous improvement through audits, surveys, and stakeholder meetings, and ensures compliance with Occupational Health and Safety standards.
Leadership is provided to employees and volunteers to foster teamwork, ongoing training, and quality client outcomes, with a focus on adherence to policies, procedures, and regulatory requirements, alongside other tasks as directed by management.
Skills and experience
To be successful in this role, it is a requirement that you have:
- Certificate III or IV Individual Support, Aged Care of equivalent work experience;
- Be willing to participate in ongoing professional development;
- Knowledge of Aged Care Quality Standards;
- Strong time management skills and a flexible attitude to post responsibilities;
- Excellent computer skills including familiarity with MS applications - word, outlook, excel and database;
- NDIS Worker Screening Check;
- NDIS Worker Orientation Module Certificate;
- Excellent Customer Service Skills;
- Current First Aid and Provide CPR;
- Australian Driver's License.
