We are currently looking for a community based Day Program Coordinator to join our QLD team. The role
This is a part - time office based role in Townsville, 3 days a week. The coordinator is responsible for coordinating and facilitating the delivery of programs which involves liaising with Support Coordinators and customers, ensuring services are provided in accordance with the customer’s assessed needs. The position may entail travelling between customer’s homes and providing individual and personalised support to customers in accordance with their support plans. Staff will perform an integral role in ensuring services are delivered to customers ethically, honestly and working within the framework of the Governing Body. The overall responsibility is to provide quality operational services, while maintaining efficiency and cost effectiveness and meeting compliance and procedural requirements.
Duties and responsibilities may include:
Facilitation of program
Rostering
Develop and manage customer services, including but not limited to schedules, service planning, intake documentation, ongoing quality assurance, confirmation and short notice placements and cancellations
Prepare and transmit schedules for customer services to support professionals
Verify allocation of support professional against customer service schedules
Assess, coordinate and monitor the relevant needs of individual customers and families in an ongoing and systematic way
Meet the specified and appropriate standards for the delivery of customer services
Ensure the ongoing development of flexible and innovative services through continuous improvement
Provide supervision, support and direction to staff
Ensure that a quality service is provided to people we support
Manage day-to-day activities of the service
Monitor the implementation of individualised plans for customers
Ensure that the rights, interests and needs of people we support are addressed effectively
Understand and respect the people we support linguistic and cultural diversity
Comply with all Just Better Care’s policies and procedures
What you will need:
Minimum Certificate IV in Individual Support or Disability Services/Equivalent
Current CPR / First Aid Certificate
Current and satisfactory Federal Police Check
NDIS Worker Screening ID/Yellow Card & Blue Card
Current Australian Manual Driver’s Licence
Reliable, registered and insured vehicle (comprehensive insurance is preferred)
Smartphone with internet access
Covid 19 Vaccination Certificate
Desirable:
Demonstrated high level awareness of issues affecting the disability, aged care and community health sectors
Experience in providing services to customers including assessment and monitoring processes
Previous customer service coordination experience in the Disability / health / aged care, community services sector
Proven relationship management with all internal and external stakeholders
