Grade 2 Dietitian- Caulfield Community Health Service (AJ1-AJ4)
Part-Time, Permanent (72 hours per fortnight)

CAULFIELD COMMUNITY HEALTH SERVICE
Caulfield Community Health Service (CCHS) provides a range of clinical and health promotion services for people of all ages who live, work or study within the local community. These multi-disciplinary services are provided in the centre, community or are home based. CCHS operates from the Caulfield Hospital site at Alfred Health.
POSITION SUMMARY:
The Dietitian will work within the multi-disciplinary Adult Health Team to provide high quality clinical services for people in the local community living at home The Dietitian will be responsible for the effective clinical management of clients with type 2 diabetes work within Diabetes Connecting Care Clinic (DCCC), and CCHS Type 2 Diabetes Service. The two services involve:
Diabetes Connecting Care Clinic: The Dietitian will work in conjunction with a Diabetes Nurse Educator (DNE), Exercise Physiologist, Pharmacist, GP with special interest, and Endocrinologist to provide an interdisciplinary approach to care, with the aim of increasing client understanding and confidence to self-manage their type 2 diabetes in the community. The Dietitian will complete assessments with the DNE and Exercise Physiologist.
CCHS Type 2 Diabetes Service: The Dietitian will work in conjunction with a DNE and Exercise Physiologist to provide an interdisciplinary approach to care, with the aim of increasing client understanding and confidence to self-manage their type 2 diabetes in the community.
The Dietitian will also hold a small case load of CCHS clients, referred for general nutrition intervention, which could include those with chronic health conditions, swallowing or chewing difficulties, malnourished, overweight or obese, gastrointestinal disorders, as well as general dietary advice. Interventions can be provided in the centre, community, via Telehealth and client's homes.
KEY RESPONSIBILITIES
Provide high quality nutrition services to support clients who live in the CCHS catchment
Provide individualised Dietetics services to clients referred to the DCCC, and CCHS Type 2 Diabetes Service.
Provide comprehensive and timely nutrition & dietetic assessment, treatment, education, referral and discharge of clients in line with professional standards and organisational guidelines
Work collaboratively with staff from other disciplines to provide multidisciplinary, holistic health care for clients. This could include referrals to other disciplines and services, to ensure a holistic approach to client care.
Plan and manage own workload to ensure adequate time for the achievement of primary objectives of the position
QUALIFICATIONS/EXPERIENCE REQUIRED
Mandatory
Tertiary qualifications in Nutrition and Dietetics or equivalent
Eligibility to be a Certified Accredited Practising Dietitian
Eligibility for DAA Membership
Demonstrated dietetic clinical experience (minimum 3 years) relevant to the Community Health sector which could include experience and skills in a range of dietetic specialities including chronic disease, diabetes management, enteral nutrition, and aged care.
Excellent communication skills, collaboration and problem-solving skills
Well-developed computer/information technology skills
Current Victorian driver's licence
Desirable
Experience working within the general medical, aged care, or Community Health sector
Experience in supervision of students
Experience in working in a Diabetes service
