About the Role
An exciting opportunity currently exists for a Grade 2 dietitian to join the Health Independence Programs team. The Grade 2 dietitian will deliver high quality person-centred, goal-orientated care within a supportive multidisciplinary team.
This position may be required to work at any of the HIP hub sites or programs. This will be at the discretion of the HIP manager and will take into consideration appropriate skills and training.
The classification range for this position is AJ1 - AJ4 ($92,627 - $104,811 per annum, pro rata for part time).
Your contribution
Qualifications providing eligibility for full membership of Dietitian’s Association of Australia and the Accredited Professional Development program
- Minimum of four (4) years post-graduate experience in a relevant field of clinical dietetics
- Demonstrated highly developed clinical management skills
- Understand and support the principles of HIP continuum of care for clients, particularly in relation to individualized, time-limited, goal-centred episodes of care that aim to improve health outcomes
- Highly developed skills of organisation, planning and problem solving
- Highly developed interpersonal skills that incorporate the qualities of excellent communication and leadership
- Current Victorian Driver’s Licence

About Department
This position is part of the Health Independence Programs (HIP). The intent of HIP is to deliver improved outcomes for clients and to support hospital demand management and flow, through delivering integrated and coordinated care to clients across the hospital and community interface, reducing duplication and fragmentation of services and ensuring client centred care planning.
SVHM HIP consists of integrated hub sites across several locations including Fitzroy, Kew, North Richmond and Fairfield.
HIP at SVHM is delivered in partnership with North Richmond Community Health (NRCH).
Community Rehabilitation is provided from our centres at Kew and Fairfield.
The key aim of Community Rehabilitation is to help people who are recovering from traumatic injury or illness, are frail or chronically ill to regain optimal function and to allow people to maximise their independence.
The position also reviews patients from the specialist clinics – Continence clinic, Falls clinic and Osteoarthritis clinic. Education to various patient groups is provided as required.
The programs offer a supportive and dynamic team environment which focusses on patient-centred care.
