You will be responsible for identifying clients for Bolton Clarke home nursing services, providing community focused nursing assessment and acting as a client advocate to achieve best possible outcomes.

- Providing a communication link between Bolton Clarke and hospital staff to optimise client health outcomes
- Promoting concepts of discharge planning through public relations activities and education
- Identifying and contributing to business development opportunities for Bolton Clarke
- Participating in relevant committees and working parties to contribute liaison service expertise
- Participating in research to increase the evidence base of discharge planning, nursing assessment and referral processes
- Bachelor of Nursing or other qualifications and/or training deemed equivalent by APHRA leading to registration as a Registered Nurse (essential)
- Minimum of 4 years’ experience (essential)
- Demonstrated clinical skills and experience in assessment and discharge planning
- Own vehicle/licence essential
