Role Overview:
This is an exciting opportunity to join Uniting SA’s Child, Youth and Family Services (CYFS) team as our FLO Case Manager. In this role, you will provide case management services and support for young people aged 12-25 years who are at risk of leaving school or disengaged from mainstream schooling to address their barriers to education, training and/or employment services
Key Responsibilities:
- Provide responsive and client-focused services, by encouraging active client participation, self-development and individual learning outcomes using a strengths-based perspective
- Provide high quality case management services to the client group of the program
- Involve clients and their supports in aspects of service including planning, services delivery, assessment, review and evaluation
- Maintaining a database, analyse data and reporting as per funding and service agreements
- Assist the Supervisor to meet program objectives, including working within the educational and vocational areas
- Provide quality programs including group work to support outcomes for young people
- Liaise with community stakeholders to plan and deliver program activities
- Develop and maintain links with other government and non-government agencies
Skills & Experience:
- Experience in working with young people from a strengths perspective, including skills in case planning and complex case management
- Experience in facilitating and planning group program sessions
- Demonstrated experience in case management including assessment, planning, referral and advocacy
- Knowledge of youth sector issues of homelessness, mental health, juvenile justice, drugs and alcohol, school engagement and retention, and young parents
- Knowledge of community resources and experience in networking and negotiating with other service providers
- Knowledge of or experience in working within the educational and vocational areas
