Crown West Medical is seeking a dynamic and enthusiastic EndorsedEnrolled Nurse or Registered Nurse to join our expanding team and to carry out our chronic care coordination, health assessments and work in our busy treatment room as required. You will need to enjoy the challenge of a fast-paced working environment and will always be completely supported by a professional friendly team of Doctors, Nurses, management and reception staff.
Crown West is a fully accredited busy, private billing General Practice with 9 GP's and a busy treatment room that provides varied high quality nursing care.
A keen interest in General Practice Nursing is a must.
Our practice is open Mon - Fri 8am - 6pm and Sundays, nursing staff operate our treatment room Mon - Fri 8am - 6pm. Our success applicant would be available to work during these hours and between 4 to 5 days per week, and hours can be negotiated.
Qualifications & experience
- Endorsed Enrolled Nurse or Registered Nurse
- Minimum two years post graduate experience (preferably in primary care and chronic health)
- Current registration with AHPRA; Current Employee Working with Children Check; Current National Police Certificate Check or the willingness and ability to obtain
- Demonstrated understanding of the needs of primary health and an interest in patient education and health promotion particularly in the area of Chronic health care.
- Demonstrated solid IT skills with attention to detail and accuracy; Ability to effectively manage patient bookings, recalls, reminders and clinical records within practice software
- Excellent written and verbal communication skills including the communication of results
- Good problem solving and negotiation skills to enable the resolution of difficult or complex issues
- Current knowledge of infection control, CPR and emergency resuscitation techniques
- Experience working to clinical governance processes and standards
- The ability to maintain a high standard of professionalism
- Preferable experience with Best Practice especially the inbuilt Health assessment templates and GP Management Plans
- Experience with Iron infusions, IUD insertion support and Ear syringing desired
Tasks & responsibilities
- Carrying out Health assessments including over 75yrs, 45-49yrs, 40-49yrs diabetic risk assessments and ATSI, Chronic care coordination including GP Management Plans.
- Assist with medical procedures and support with patient care/management as required
- Address telephone enquiries, triage and patient assessment
- Sterilisation of equipment
- Maintain regular stock levels according to practice procedures; Ordering medical supplies; monitoring medication expiry dates; maintenance of oxygen and medical emergency supplies
- Clinical data management; attending to recalls & reminders in a timely manner and maintaining patient documentation in a clinically effective manner
- Maintaining Infection Prevention and Control
- Maintain awareness of current and new legislation to ensure the practice complies with all regulatory and statutory obligations
- Maintain regular checking of the environment and equipment to ensure safe, efficient and effective practice is adhered to
- Maintain patient and practice confidentiality at all times
- Working as a team with our other Practice Nurses
