HCP Junior Care Manager
Amazing Procare is a rapidly expanding Aged Care and NDIS service provider based on the Gold Coast, across the Tweed Coast and Brisbane South areas in QLD and the Central Coast and Hunter Region in NSW. Working full time at our office in Nerang, you will be someone who can be relied upon to do what’s best for the person benefiting from your service. You will be someone who is attentive, and who will be proactive in supporting, noticing and reporting changes in physical health and emotional well being.

About the Role
- Manage a caseload of consumers with complex needs through client meetings & assessment, referral, review and goal planning
- Managing client funds and budgets
- Building strong internal & external relationships including client's families, allied health providers and support workers
- Identify and grow new business opportunities through promotions/expo’s/networking
- Provide appropriate support to culturally & linguistically diverse clients
- Ensure compliance with relevant legislation
- Part of this role is to engage with other referral partners (clinical nursing/allied health/my aged care etc)
- Ability to be able to travel as far as the Sunshine Coast and surrounding regions on occasions if required (This could lead to a monthly/bi monthly requirement as the business grows in these regions, all expenses paid)
Preferred Attributes
- Tertiary qualifications (Diploma in Community Services or Cert. IV in Aged Care) is ideal but not essential
- Previous experience in Aged Care or Community Services in Care Management is ideal but not essential
- Be a natural problem solver and creative thinker with the ability to get the very most out of the package for your client.
- Contribute to ideas and continuous improvement for the company's performance growth.
- Demonstrated understanding of the needs of older people and their carers
- Resilient and able to work effectively under pressure and in an ever changing environment
- High attention to detail with focus on achieving positive consumer outcomes
- Demonstrated experience in case management, including managing government-funded care packages and developing budgets
- Demonstrated ability to create and implement consumer-directed services
- Well-developed communication (written and verbal), interpersonal, problem solving, client database management and time management skills
- Team oriented with an open and collaborative work style
- Current driver’s licence and insured vehicle
- National Police Clearance/NDIS Check
