Tell me more...To work as a member of the Pop-Up Palliative Care Service, at an advanced level with minimal supervision. To assist with the delivery of Palliative Care led, consumer centred model of care that specifically supports patients and their families through care co-ordination, advocacy, and education around palliative care and end of life care. To work in partnership with communities and a range of service providers to develop and implement culturally safe Palliative Care services for Aboriginal and Torres Strait Islander peoples from culturally and linguistically diverse backgrounds with special or diverse needs.
To be successful in this role you will have:
- Ability to work at an advanced level with minimal supervision within a multidisciplinary team for Palliative Care Services, caring for patients and families from remote communities from Cape York
- Ability to deliver Palliative Care Services within a primary health care context in response to the health needs of individuals and families.
- Appropriate clinical skills for the delivery of evidenced based health assessments for Palliative Care promotion/education, activities in response to the needs of Aboriginal and Torres Strait Islander families.
- Possess exceptional interpersonal communication skills, including community and client liaison, negotiation and networking with groups and organisations.
- Base comprising salary between $2722.20 - $2910.80 per fortnight
