About the Role:
In the role of HCP Service Coordinator, you will be instrumental in coordinating the delivery of services to our HCP clients. Your responsibilities will include planning, implementing, and managing all aspects of our Home Care Packages to meet and exceed the expectations of our clients and their stakeholders. This position offers the opportunity to make a significant impact in the lives of those we serve, ensuring they receive the support they need to live independently.
Key responsibilities include:
- Conducting assessments to identify client needs within the Home Care setting.
- Managing and overseeing individual client budgets.
- Assisting clients in navigating through the HCP services landscape.
- Delivering effective, goal-oriented, and outcome-focused services.
- Advocating for and promoting the rights and choices of our clients.
- Full case management, from inception to completion.
About You:
You are a compassionate individual with exceptional communication skills, capable of engaging with a diverse range of clients, families, and service providers. Your attention to detail and ability to manage priorities will ensure the delivery of quality services within deadlines. With a proactive approach and the ability to solve problems effectively, you will deliver tailored solutions that best suit our clients' needs.
Your background includes:
- Experience in administering Home Care Packages, Consumer Directed Care, or similar services to the aged, frail, and/or disabled individuals.
- A proven track record of coordinating, monitoring, and reviewing community-based services, including the development of Care Plans.
- Familiarity with integrated management systems in a continuous improvement setting is desirable.
- A comprehensive understanding of both government and non-government health services is preferred.
This role is open to candidates with direct experience in Home Care Packages.
Eligibility to work in Australia is a must, and successful candidates will undergo pre-employment screening.
