The Home Care Package Program Nurse Role
Country Health Connect (CHC) has moved to a Consumer Directed Care model for the provision of aged care, disability and support services. This allows consumers to maintain greater control over their lives by allowing them to make choices about the services they receive and who delivers those services. Consumers are encouraged to actively participate in identifying and setting goals, and to determine what level of involvement they want in managing their own living arrangements.
Under this model of care, the funding associated with the role of the Registered Nurse (Home Care Package Program) is controlled by consumers, rather than by the health provider. This means that consumers have an active role in determining the shifts that are available, the duties that are performed, the frequency of demand for the service, and the choice of provider organisation.
Employees classified at this level provide nursing services in health service settings and autonomously in the community. They will work closely and collaboratively with the consumer, their family and other health professionals, to provide quality outcomes and maintain consumer independence where possible. Employees working within this level develop from novice practitioners to a proficient level of professional practice. They consolidate knowledge and skills and develop in capability through continuous professional development and experience. An employee at this level accepts accountability for his or her own standards of nursing care and for activities delegated to others.
Must be registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
Country Health Connect focuses on the delivery of exceptional care to those living within our communities who require some support to remain living independently or reside in one of our residential aged care facilities.
Our professional and caring staff deliver health care and wellbeing support across regional South Australia in hospitals, local health services, community centres, residential aged care facilities and clients’ own homes.
The South Australian public sector promotes diversity and flexible ways of working part-time. Applicants are encouraged to discuss the flexible working arrangements for this role.
Appointment will be subject to a satisfactory Criminal History Check and appropriate immunisation requirements.
