This local NFP works to build an inclusive, resilient, self-reliant and creative community. Working with people from all walks of life, they provide community support services to the local community and in particular to disadvantaged groups such as the aged, people at risk of homelessness, people with mental illness, people from culturally and linguistically diverse backgrounds and people on low incomes.
Role;
Based in Sydney's Inner West, we're looking for an experienced and enthusiastic Case Manager to support people who are at risk of homelessness and social isolation, particularly those who are living in Boarding Houses, and those who may be ageing, living with disabilities and socially isolated.
You will work collaboratively in a program supporting formerly rough sleepers with 2 years of wrap around case management support, helping individuals deal with the issues which led to their initial homeless status, and build on their independent living skills whilst working towards permanent housing stability.
You will:
- Deliver person centred, trauma-informed and culturally safe case management
- Conduct needs assessments and develop collaborative client led case plans
- Advocate for the best possible outcomes for clients and support their capacity to self-advocate
- Conduct formal and informal client reviews to evaluate strengths, progress and emerging needs and goals.
Criteria;
- Tertiary qualifications in Community Services, Social Work, or similar field
- Demonstrated experience working with disadvantaged and marginalised people experiencing homelessness
- Demonstrated experience in needs assessment, case planning and exit planning
- Trauma-informed practice experience
- Based in Sydney's Inner West
- Permanent, full-time role
- Must hold a full unrestricted driving license
- Must provide a NSW WWCC
- Must provide a recent National Police Check (within the last 6 months)
- Must be fully vaccinated against Covid-19
