The Opportunity
We are looking for an Intake and Client Liaison to join our supportive and professional multidisciplinary teams in a flexible part time capacity.
Responsibilities include but are not limited to;
- Act as the initial contact point for new SSG clients providing general information and how to access Social Support Groups
- Register clients and maintain SSG waiting lists, collecting initial information for referrals, complete client assessments
- Working with Social Support Group Manager, support the achievement of program targets through monitoring group capacity and support filling of program vacancies to maintain capacity
- Monitor portals and waiting lists to maximise groups.
- Identify gaps is Social Support Group model, interest and needs for clients and elevate these to the Manager for adaptation of the Social Support Group model
- Ensure accurate and concise data collection
- Prepare progress reports for program managers and exec, highlighting key insights, impact and outcomes
- Coordinate the administrative operational requirements and systems in collaboration with management teams to ensure programs are inclusive, flexible, compliant
- With Social Support Group Manager, identify opportunities to integrate, innovate and work collaboratively across teams
Skills & Experience
- Minimum of 2 years’ experience in community and/or health related or centre based activity programs.
- Conflict resolution and problem-solving abilities.
- Strong interpersonal and communication skills.
- Empathy and the ability to connect with individuals from diverse backgrounds.
- Good interpersonal and communication skills, both written and oral
