Deliver intensive, clinical assessments, interventions and support to people who are at risk of frequent relapses in their mental health and recovery, whom without intensive clinical interventions, would result in them requiring frequent hospitalisations to an acute inpatient service.

About the role
This position will work in collaboration with other community clinicians to reduce emergency department presentations, readmissions and overall lengths of stay for the identified consumer cohort.
Intensive Support Clinician – Senior Health Practitioner $124,435 - $133,901 per annum
Clinical Nurse Consultant Intensive Support Clinician – $132,562 - $143,924 per annum
