The Role:
The NDIS Service Coordinator is responsible for planning and leading support for people with disabilities within their home, shared accommodation and in the community to enable them to achieve their goals and live fulfilling lives. NDIS Service Coordinators lead a team of support workers responsible for maintaining an environment that empowers people with a disability.
Reporting to the Client Service Manager - you will:
- Ensure support to a small caseload of individual clients in their homes is delivered in a manner consistent with the values of InLife Independent Living
- Mentoring, providing leadership on a day to day basis to the team
- Liaising with and developing relationships with external stakeholders including client's families, allied health professionals and support coordinators
- Data reporting in line with individual plans
- Day to day financial management
- Client health and wellbeing tracking, ensuring follow up and individual appointments
- Supporting clients to work towards goals
- Using Workplace to share and communicate information for each client team
- Ensure the team maintain all required reporting
- Managing/maintaining the physical premises and as required activities associated with the house.
- Undertaking shifts doing direct support work as per client plans and in consultation with your manager
- Hold regular client team meetings according to client needs

- possess a high level of integrity, passion and reliability
- are an advocate for independence
- have a positive track record supporting people with complex presentations including intellectual disabilities, complex health and physical disabilities
- Possess Relevant qualifications and/or experience in Community Services/Case Management (or equivalent)
- are experienced in leading a team and supervising the work of others
- possess a current Victorian Driver’s License
