About us
Here2Home (formally From Here2Home) are an ethical NDIS service provider of Support Coordination and Specialist Support Coordination. We aim to support people living with a disability to achieve personal goals and address barriers they may be facing, whilst providing compassion to each individual and their experience.
We currently have room for some new faces on our team in Adelaide! This includes both Level 2 Support Coordinators and Level 3 Specialist Support Coordinators.

Qualifications & experience
- Minimum of 2 years NDIS support coordination experience is necessary to apply for this role
- Pay is considered based on level of SC applying for. Level 3 Specialist Support Coordinator role will require an allied health degree, and subsequently pays at a higher level. Level 2 Support Coordinator does not require a degree but will require relevant experience.
- Knowledge of various disability types and the National Disability Insurance Scheme (NDIS) is required in this role
- Must have or be willing to obtain a Department of Human Services Disability Services Employment Screening check or NDIS Workers Clearance, in conjunction with a Working With Children Check (essential)
- Current Australian driver's license with a roadworthy vehicle (essential). Must be willing to use own vehicle for work related use, including home visits and attending stakeholder meetings. You will be paid for your travel time.
Tasks & responsibilities
- Writing formal reports to a high standard
- Liaising with various health professionals, non-government agencies and government departments
- Ability to prioritise and coordinate services for a complex case load
- Resolve points of crisis, and addressing barriers.
- Ability to work autonomously and in an organised fashion
- Ability to build rapport easily with others
- Coordinate and budget services from an NDIS plan
