Form of workThis is an exciting new opportunity within the Pathways to Home (PTH) program which sits within the broader Western @ Home service.
Pathways to Home program is a transitional care program for people with disability to address hospital discharge barriers. Many patients with a disability under the age of 65 face barriers to timely hospital discharge. This cohort no longer has a medical requirement for hospitalisation but is unable to be discharged safely until their longer-term support arrangements are finalised under the National Disability Insurance Scheme (NDIS).
Safe discharge may require any combination of supports for which the individual does not currently have approved access, including health supports, disability supports, accommodation, home modifications or assistive technologies/equipment.
The Grade 4Occupational Therapist) in the Pathways to Home will be responsible for the specialist assessment, care and management of Pathways to Home patients, as well as providing expert contribution to service performance and outcomes. The Grade 4Occupational Therapist will have a clinical caseload and key leadership role within Pathways to Home and the broader Western @ Home team; working collaboratively with key stakeholders and other service providers.
This position is offered on a fixed term, part-time basis.
To be Successful in this Role, you will:
- Have at least 10 years’ relevant clinical experience
- Hold a post-graduate qualification in relevant field, or working towards
- Have highly developed leadership skills and experience leading teams
- Demonstrate highly developed communication and interpersonal skills
- Ensure safe and high quality patient care is delivered in line with Western Health values and Live Best Care framework
- Contribute to a culture that values ongoing learning and be part of a positive workplace
- Hold a valid Victorian Drivers Licence
