We have an exciting opportunity for an Operational Support and Clinical Facilitator to join our dedicated Central Office team based in Glenside.
About the role
The Operational Support and Clinical Facilitator works closely with our organisational services (Residential & Community) managers/teams to ensure high quality, safe and person centred care and services are delivered to residents/clients.
The role is a hands on leadership role, responsible for best practice in:
- Clinical and care outcomes through the monitoring of Southern Cross Care clinical care and medication management software, responding to clinical/care risks, and leading change.
- ACQSC readiness across our homes and services.
- Provision of Support and advice to the Service Managers to address complex resident/client clinical and care needs in line with the ACQSC standards.
- Improvement of systems/documentation related to clinical and care needs in line with the ACQSC standards.
- Undertaking root cause analysis of issues to prepare a timely plan action to improve resident/ client outcomes.
- Formal/informal mentoring and educational support to all staff where gaps have been identified in any aspects of care and service delivery.
- Leading and promoting a positive learning and engagement culture for Southern Cross Care.
- Leading and promoting a strong organisational commitment to early intervention, healthy ageing and best practice.
- Supporting clinical teams to apply clinical reasoning, clinical judgement, critical thinking when monitoring care and reporting on resident/client outcomes in line with ACQSC standards.

About you
We are looking for people who will be the difference in the lives of our residents and clients.
The successful candidate will have the following skills and experience:
- Bachelor of Nursing or equivalent including current AHPRA registration as a Registered Nurse.
- Demonstrated experience in a Clinical Nurse or Quality role.
- Excellent understanding and working knowledge of ACQSC standards including expectations and outcomes.
- Experience in the leadership of staff through periods of change in culture, services and approaches to resident care.
- Demonstrated experience in the leadership of direct care staff including experience working from a person centred Healthy Ageing approach.
- Sound interpersonal skills and the ability to express ideas clearly and concisely
- Effective problem solving, negotiation and decision making skills.
- Demonstrated competence in adult learning principles.
- Knowledge and experience of clinical documentation and continuous improvement within residential aged care would be highly desirable.
- A current Drivers licence is essential.
