About the roleThe Program Manager, Respite & Restorative Services is responsible for the team management and overseeing the service delivery at Myrtle Cottage, The Pines Cottage, Willow Fern Cottage, Goolwa Community Hub & the Buddy Program.Key responsibilities of this role include but are not limited to;Key functions include:
  • In partnership with the Restorative Care AHP or RN (where it is clinically indicated) complete comprehensive, person-centred clinical & non clinical assessments and scheduled care plan reviews for clients and their caregivers.
  • Monitor and evaluate each respite service to ensure the delivery of a high quality respite service model that focuses on the reablement and wellness of clients.
  • With the Restorative Care AHP or RN respond to client incidents and identify early signs of decline to provide best practice clinical support to staff, including the Respite Site Leaders, Respite Assistants & Buddy Support Workers.
  • Support and encourage respite staff to find creative solutions to overcome barriers to client participation in meaningful activities in order to help achieve their wellbeing goals.
  • Monitor client & caregiver satisfaction, successful achievement of wellbeing goals and respond to feedback.
  • Maintain an engaged, cohesive respite team while monitoring staff performance and job satisfaction.
  • Partner with internally and externally for referrals and care integration across multiple programs including SCC
Community Health and Wellness, Home Support Services and Residential Care.For further details regarding the scope of the role, please refer to our website or the attached job description.About youWe are looking for people who will be the difference in the lives of our residents and clients.The successful candidate will have the following skills and experience:
  • Certificate in Aged Care (III or IV)
  • 5+ years of experience working in Aged Care
  • Current drivers license
  • Experience in aged care health and community services, clinical case management and collaboration.
  • Ability to provide leadership in the management of community aged care staff and services to a range of client groups and working with multi-disciplinary teams and health professionals.
  • Demonstrated ability to lead and contribute to a high performing team.
  • Ability to provide leadership and motivation to staff to provide quality client focused services.
  • Demonstrated change management skills with the ability to manage staff and the site through the stages of change.
  • Demonstrated ability to deal with complexity and ambiguity, and with multiple and often conflicting priorities, creating clarity and a basis for productive action.
  • Competency with current aged care service, acts and programs and an awareness of current best practice models of care, and an ability to integrate these for the greatest client benefit.
  • Demonstrated team and program management
