In this position you will:
1. Assess and prioritise clients’ welfare needs while undertaking appropriate interventions.
2. Provide information, assistance and support to clients, families and/or carers as required.
3. Provide advice on eligibility for benefit entitlements, assist with applications and liaise with Centrelink and other government agencies to ensure entitlements are received and maintained.
4. Develop/maintain welfare resources and providing updates to case managers regarding accommodation availability and referral processes.
5. Work with the business development manager and stakeholder, to onboard new residents, ensure that care needs are met and changes in client functioning are reflected in both, funding and services provided.
6. Provide learning, coaching and support to Direct Support Workers and practice/team Leaders and others to improve assessment and support outcomes for clients.
7. Provide leadership which ensures delivery of each client’s NDIS goals using a Person-Centred Active Support framework by actively supervising each location.
8. Assist with the development and maintenance of client related documentation.
9. Complete documentation such as reporting to the NDIS Safeguard and quality commission of any notifiable incident and restrictive practices and to allow client services to be evaluated as per management plans and NDIA requirements.
10. Ensure all incident reporting and NDIS commission processes are completed in line with legislative timeframes.
11. Ensure that all restrictive practices are documented, reported, and supported by the relevant management plans required.
12. Assist participants to develop and review goals prior to their new plan. Lead the team to deliver Person Centred Active Support according to Glory Care’s practice framework, policies, and procedures.
13. Mentor individual support workers in their professional development within the Glory Care practice framework and ensure the team’s continuous improvement in the delivery of supports to clients with complex needs.

Take reasonable care for the health and safety of self and others who may be affected by their actions at work and to comply with relevant legislative and policy WHS requirements Advocate for clients, providing the right amount of liaison between families, guardian, community groups, healthcare professionals, the NDIA and other services in everyone’s life.
