Alpha Omega Management Consulting Pty Ltd is looking for a full-time/part time Registered Nurse to work for in-home aged care and disability services.
The role is based on-site in Cabramatta Office with home visits to clients.
The Registered Nurse (RN) is responsible for the provision of high-quality client services through assessment, referral, monitoring and clinical service management and in liaison with other team leaders as appropriate, in area of responsibility. This may be achieved through visiting clients at their home, developing a care plan and implementing service delivery.
Job description:
- Deliver holistic quality nursing care in accordance with the Nurses industry, National Competency Standards competencies, NSW Health policies and procedures, to achieve patient/client health outcomes.
- Communicate effectively with clients to provide an understanding of service delivery processes and clinical interventions, to ensure client needs are identified and requirements are met.
- Consult and liaise with Registered Nurses within the HCP and/or NDIS team to ensure continuity of care to patients/clients.
- Maintain clear and concise client records that meet legislative requirements to ensure continuity of safe patient care.
- Utilise a systematic approach to assess, analyse and propose alternative solutions to situations that arise, in order to resolve patient care issues, within the scope of professional practice and under the supervision of the Registered Nurse team leader.
- Plan and prioritise own work requirements effectively to meet defined patient/client care objectives within agreed timeframes.
- Maintain up to date knowledge and skills, and practice within the risk management, safety, and quality frameworks as applicable to ensure the health and safety of staff, patients and visitors.
- Maintain responsibility for personal and professional development by participating in training/education activities, and performance reviews/appraisals in order to continuously improve the level of service provided to patients/clients.
- Assisting the team leaders in guiding and monitoring care worker competency of high impact clients.
- Overseeing the care delivered by non-clinical coordinators including health assessment, care planning and practice support to Case Managers/Care Coordinators.
- Ensures proper documentation of clinical meeting minutes and appropriate follow up and recommendations is adhered to in intervention plan.
- Ability to speak fluently English and Mandarin/Cantonese is advantageous.
- Working toward APHRA registration for Nursing or having current APHRA.
- Have experience in aged or disability field.
- Committed to providing exceptional customer service across all channels – written, phone and face to face.
- Ability to communicate clearly and concisely, varying communication style depending upon the audience.
- Excellent attention to detail and written skills when communicating with others, both internally and externally.
- Willingness to assist and support others as required and get on with team members.
- Positive approach to change (essential).
- Full driver licence, and accessible to a vehicle.
- Current valid police check / NDIS Worker Screening Check or willing to obtain it as a pre employment requisite.
- Working with Children Check (or willing to obtain it prior to commencement).
