About the Role:

An exciting opportunity is presented to work within the Northern Adelaide Local Health Network as a Nurse Unit Manager within the Division of Medicine.

This position is responsible for leading, influencing, planning, and supporting the design, implementation, and daily operation of the Safe at Home (S@H) Program.

The NALHN Safe@Home Program aims to provide in-home monitoring services to patients to help them manage their health safely in their own homes, and to reduce preventable presentations at LMH and Modbury Hospital Emergency Departments.

Vision: To enable patients with chronic disease to manage their health safely in their own homes through individualised digital monitoring, with appropriate support from healthcare providers.

Aims: The Safe@Home program aims to positively impact specific patient cohorts, including:

  • Chronic Heart Failure
  • Chronic Respiratory Disease
  • Diabetes Mellitus
  • Chronic Renal Failure

Safe@Home is based on the strategic imperative of Service Design under the NALHN Strategic Plan 2020-25. Under this imperative, we deliver services tailored to meet the health needs of our population in the most appropriate, effective, and efficient way.

In line with the imperative of being Digitally Smart, technology is an enabler for this approach. The program will initially be set up using the iCCnet SA (Integrated Cardiovascular Clinical network SA) platform and hardware.

The S@H Nurse Unit Manager/Project Lead will utilise specific clinical subject matter expertise and clinical leadership to actively engage stakeholders to ensure successful adoption, service delivery and continuous improvement of the S@H Program.

In this role you will use your advanced clinical knowledge and extensive experience to provide pivotal co-ordination of patient/client care delivery initiatives in a patient/client care area.

The primary focus of this role is the direct line management, coordination, and leadership of nursing/midwifery and/or multi-disciplinary team activities to achieve continuity and quality of patient/client care and drive successful outcomes of the S@H Program.

As the Safe@Home Nurse Unit Manager/Project Lead you will accept accountability for the outcomes of nursing/midwifery practices and/or multidisciplinary outcomes in the specific practice setting; for addressing inconsistencies between practice and policy; and for developing team performance and a positive work culture in the interest of patient/client outcomes.

The position is also responsible for providing advice regarding the development or amendment of Local Health Network operational policies to support new S@H related processes and work practices.

To be eligible for this position, you must be registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia and hold, or be eligible to hold, a current practicing certificate.
