Preventative Integrated Care Service (PICS) is an innovative community-based service providing rapid access (review within 24-48 hours) to intensive specialised medical management, delivered by a team of medical, nursing, and allied health clinicians. We focus on supporting people with chronic conditions who require multi-disciplinary input to avoid a potential hospital presentation or admission.

About the role
The Preventative Integrated Care Service (PICS) will provide a rapid response (review within 24-48 hours) for intensive sub-acute medical management, delivered by a specialised medical, nurse practitioner and allied health team with a disease specific focus through flexible and varied modalities.
The Pharmacist Senior will provide a high-level clinical pharmacy service to patients of this service. The position functions with a high degree of independence providing clinical intervention and advice, workflow organisation and service developments. This role offers the successful applicant an opportunity to be an integral part of a dynamic rapidly expanding service.

About you
Our ideal candidate will be someone who can demonstrate the following:
- Honesty, integrity and respect for all patients, carers and staff.
- Excellent communication skills to facilitate service provision and development within legal frameworks and via virtual care platforms.
- High level of clinical knowledge, expertise and skill in contemporary Clinical Pharmacy commensurate with years of experience
- Knowledge and clinical skills required to deliver a high level ambulatory pharmacy service.
- Ability to initiate, plan and evaluate local service improvement and provide clinical supervision to personnel. History of working effectively with other people as a member of a team to achieve shared goals.
- Knowledge or the ability to rapidly acquire a broad knowledge of current policy of Queensland Health regarding supply of medications, Commonwealth programs and State initiatives involving safety and management of medications and how to apply those policies in clinical practice.
- Ability to utilise skills in providing guidance to others, knowledge of where to locate information, how to evaluate its validity and to communicate that information effectively in the professional development of students and less experienced staff
Mandatory Requirements:

- Appointment to this position requires proof of qualification and registration or membership with the appropriate registration authority or association within Australia such as a Pharmacy tertiary degree that is recognised by the Australian Health Practitioner Regulation Agency, as conferring eligibility for registration. Certified copies of the required information must be provided to the appropriate supervisor/manager, prior to the commencement of clinical duties.
