We currently looking for Service Delivery Coordinator:
The role is responsible in developing and maintaining strong, positive relationships with individuals we support and their families. This partnership approach will help to set and achieve goals in a person and family-centred way.
Duties may include, but are not limited to:
- Demonstrate understanding and respect of individuals rights to exercise choice and control in the delivery of supports.
- Ensuring the plans meet all Qualitative Criteria for the Quality Management Framework and all reporting requirements to NDIA
- Reviewing and monitoring individual plans, seeking regular feedback from staff and the individual, their family and carers to update or change strategies where required.
- Provide mentoring and coaching to support staff to ensure delivery of high quality services
- Providing supports to participants
Essential Requirements
- Demonstrated values driven and person centred-approach to provide support for people with disabilities, their families and carers
- Demonstrated knowledge and understanding of the National Standards for Disability Services
- Experience in planning, coordinating and monitoring supports and services for people with disabilities, their families and carers
- Demonstrated ability to lead a work group, set priorities and monitor work flows
- Demonstrated high level of verbal, written and / or communication skills
- Demonstrated ability to manage budgets
- Relevant qualification in Human Services, Community Services or Disability.
- A satisfactory National Police Clearance that is less than three months old.
- NDIS Worker's Screening (or willingness to obtain).
