Moorditj Koort Aboriginal Corporation (MKAC) is an Aboriginal community controlled, not-for-profit organisation supporting the health and wellness of Aboriginal and Torres Strait Islander peoples living in Western Australia.
The Vision is to contribute to healthy and flourishing Aboriginal communities across WA by providing access to a holistic one-stop-shop for health and wellbeing support.
Position summary:
The Transition Care Program is seeking an experienced, innovative and flexible individual to work as a member of the TCP Team.
The program aims to assist Elders to reach their maximum functioning capacity and to help determine their long-term care arrangements.
A successful applicant will need to demonstrate sound clinical judgement, assessment and practice skills, and have an understanding of the issues affecting patients with complex functional and cognitive needs and an interest in aged care issues. They will also need to be highly motivated, enthusiastic and be able to work both independently and as part of a team.
The successful candidate must have the following essential certifications.
- Relevant tertiary qualifications in social work, psychology, sociology or related field
- Current registration with AHPRA
- WA Valid Driver’s license
- Police Clearance no more than 3 months old (Mandatory)
- Working with Children check
- First Aid or willing to obtain.
- COVID-19 Vaccination Certificate
Skills, Abilities and Knowledge
- Formulation of treatment plan with clients and family to assist them to navigate services.
- Provides culturally secure case management for clients and their family members through the provision of advocacy, follow-up, referral to appropriate services and collaboration with other support agencies.
- Provides appropriate therapeutic intervention/s as required.
- Provides brief intervention counselling, support and advocacy as relevant within scope of experience.
- Participate in case management of clients with other service providers where appropriate.
- Participate in the planning and development of community development and capacity building activities.
- Liaise with health providers, other government and non-government agencies and communities.
- Assist with the provision of culturally secure education and training to service providers and community members with respect to the inter-generational impacts of Stolen Generation policies.
- Maintain confidential client records and provides timely relevant statistical information.
