As a NDIS Specialist Support Coordinator, you will play a crucial role in helping our clients navigate the National Disability Insurance Scheme (NDIS) and access the support they need.
Position Overview: We are seeking an experienced, dedicated and compassionate Specialist Support Coordinator to join our team. The successful candidate will be responsible for providing expert guidance and support to individuals with complex disabilities and their families, helping them to understand and utilise their NDIS plans effectively.
SSCS WA specialises in supporting individuals with complex clinical needs, justice interface and psychosocial disabilities Australia wide. We have a large clientele base in regional Western Australia. We are passionate in ensuring that individuals residing in remote locations have the same high quality of interface with specialist clinicians and access to support providers and accommodation.
Responsibilities:
- Work closely with individuals with complex disabilities and their families to understand their needs, goals, and preferences.
- Provide comprehensive support in navigating the NDIS system, including plan development, implementation, and review.
- Conduct thorough assessments to identify strengths, challenges, and support requirements.
- Collaborate with external service providers, community organisations, and government agencies to coordinate and access appropriate supports and services.
- Empower clients to build their capacity for self-management and advocate for their rights and preferences within the NDIS framework.
- Maintain accurate records and documentation in accordance with NDIS guidelines and organizational policies.
- Stay informed about changes and updates to the NDIS framework and relevant legislation.
Requirements:
- Bachelor's degree in physiotherapy, occupational therapy, psychology, specialist teaching, or other allied health professional.
- Demonstrated experience working with complex individuals with disabilities and their families in a Specialist Support coordination or similar role.
- In-depth knowledge of the NDIS framework, including eligibility criteria, funding options, and service delivery models.
- Strong communication and interpersonal skills, with the ability to build rapport and establish trust with clients and stakeholders.
- Excellent organisational and time management skills, with the ability to prioritise tasks and manage competing priorities effectively.
- Commitment to upholding the rights, dignity, and independence of individuals with disabilities.
- Current driver's license and access to a reliable vehicle.
