The Support Coordinator will support NDIS participants, their family and carers in identifying and connecting with appropriate providers of supports with a view to overcoming barriers, developing skills and achieving the objectives of their NDIS Plan.
You will:
• Build and maintain relationships with key external stakeholders, particularly Local Area coordinator.
• Support NDIS participants to implement their NDIS plan, including assistance to identify and select providers ensuring to maintain clients choice and control.
• Work creatively to find positive outcomes.
• Deal with crisis situations.
• Optimise participant plans to ensure participants get the most out of their funded supports and plan expenditure remains within budget.
• Strengthen and enhance participant abilities to self-direct their own supports.
• Assist NDIS participants in planning ahead to prepare for their NDIS plan review.
• Ensure clear, accurate, and timely record keeping.
• Provide orientation and training to new Support Coordinators and ensure staff onboarding and HR functions are completed.
• Supervise and lead Support Coordinators, to ensure the staff’s optimum development and success in the role.
• Assist with annual performance appraisals.
What you will need:
• A relevant qualification in community services, welfare support, disability support or social work.
• Demonstrated leadership and management experience, ideally with oversight of a small team.
• Previous experience working with children, young people and their families in a case management or service coordination role.
• Knowledge of community resources and disability rights issues.
• Strong working knowledge of the NDIS and NDIS Support Coordination.
• An understanding of Child Safe Standards.
• A professional and ethically minded approach; and commitment to supporting and promoting the individual needs, skills, abilities, and personal goals of people with disabilities.

Screening and external authorisation requirements:
• Employee Working with Children Check
• Current NDIS Workers Screening Check
• Right to work in Australia
• Applicants who have lived overseas for 12 months or longer during the past 10 years are required to provide the results of an international police check.
• Completion of NDIS Quality and Safeguards Commission Worker Orientation Module ‘Quality, Safety and You’, and provision of a copy of the certificate of completion.
• Completion of New Worker NDIS Induction Module and provision of a copy of the certificate of completion.
• Current full Australian Drivers Licence
• Your own vehicle (registered & insured)
• A current First Aid Certificate (HLTAID01) and CRP (HLTAID009)
• Completion of a pre-employment injury and illness declaration.
