Support Coordination Team Leader

YCS offers multiple services including Support Coordination, Short-Term Accommodation, Community Access, Supported Independent Living, In-Home and Daily Living Skills Hub. 

The Role: 

We are looking for an experienced Team Leader to join us. Our Support Coordination services are growing, and we require a passionate leader to effectively support the team.

We're committed to high quality service delivery with demonstrated leadership experience along with consideration to a person centered and individualised approach.

We are seeking an innovative, a solution-focused leader to join our team. This person must be able to with the strengths of their team members whilst upholding the team model when implementing holistic supports within the persons NDIS plan for the purpose of building capacity.

The successful applicant will be a full-time (negotiable) who will work onsite in our Springfield, QLD location (non-negotiable).

Key Responsibilities include: 

• Lead and supervise a team of support staff, providing guidance, training, and support to ensure high-quality care and support services.
• Support a positive team culture and promote a sense of teamwork
• Hold a positive and proactive attitude and set an example for your team. 
• Provide Support Coordination duties in line with people's goals and preferences when developing and implementing individual actions plans for your participants. 
• Collaborate with internal and external stakeholders to develop and implement innovative strategies to improve service delivery and outcomes. 
• Prioritise, re-prioritise and delegate tasks amongst the team.
• Ensure compliance with all relevant policies and procedures, including health and safety regulations, privacy and confidentiality policies, and quality assurance standards.
• Monitor and evaluate the performance of support staff, providing feedback and coaching as needed.
• Maintain accurate and up-to-date records and reports, including participant progress reports, staff performance evaluations, and financial reports. 

Key Selection Criteria:

• Post Year 12 Qualifications
• Demonstrated understanding of Qld Disability Services Act 2006, NDIS Quality and Safeguards Commission, NDIS Practice Standards, NDIS Pricing Arrangements and NDIS Reportable Incidents (including authorised and unauthorised Restrictive Practices)
• Minimum 2 years’ experience in leading teams, including supervision, performance appraisals, training and development.
• NDIS Orientation Module, First Aid/CPR, NDIS Worker Screening and Working With Children Check
• Open Driver’s License 
• (Preferred) Experience working within the Mental Health sector
• Demonstrated effective written communication, including report writing, shift notes, email etiquette with internal and external stakeholders
• Demonstrated ability to adapt communication and leadership style to get the best out of team members based on their individual strengths
• Demonstrated effective Stakeholder Engagement to ensure implemented supports are holistic for the person supported by YCS 
• Demonstrated ability to work autonomously, but most importantly in a close team setting to ensure responsive support provision and continuity of supports

• Ability to manage time and prioritise tasks to meet deadlines
