Support Coordination:
- Client engagement- acting as a point of contact.
- Build rapport, negotiate, develop and/or maintain mutually beneficial relationships with a diverse range of stakeholders, Local Area Coordinators or other NDIS Providers to achieve organisational and client objectives.
- Organise services for each individual client including providing a person-centred assessment and care plan.
- Consult with clients, carers, and families to facilitate service planning.
- Working closely with the rostering team to ensure clients services are scheduled as per their services agreements, and advising in advance of any changes to schedules.
- Develop and monitor NDIS Service Agreements.
- Negotiate services to be provided with prices when developing service agreements and creating service bookings with preferred providers.
- Plan and/or regularly review client funding and budgets.
- Maintain ongoing communication with clients, carers, and families to ensure they are aware of rights and responsibilities to make informed choices.
- Manage risks within principles of Positive Approach to Challenging Behaviours
- Work within legislated NDIS frameworks.
- Administer and coordinate the intake of a client from the initial contact through to commencement of services
- Guide client and supporters through the intake process, assisting to follow the processes for NDIS planning
- Develop and maintain positive relations with clients during the intake process including up to and including the service agreement process
- Ensure ACALS collateral is maintained with the most relevant intake information, in collaboration with the Director
- Coordinate and facilitate Intake meetings
- Liaising with clients to complete all paperwork required for intake
- Prepare reports and documentation on current and predicted demand.
- Management of CRM
- Use and update software system and technology for wellbeing updates
- Manage and resolve client, caregiver and external advisor emails and phone calls.
Administrative Support:
- Inputting meetings within calendar, such as: required and due tasks, client and provider meetings, any other important projects.
- Greet visitors and offer hospitality. Advise relevant staff to arrival of visitor or guest.
- Organise and maintain kitchen supplies as needed.
- Use Microsoft Office Suite (Word, Excel, PowerPoint, Outlook)
- Answer and direct incoming calls.
- Maintaining and updating files within companies secured online file share.

- Inform Accessible Care and Lifestyle Services of any conditions that may impact or limit ability to carry out responsibilities required of the role.
- Be able to use a computer, use mobile devices, undertake manual handling, lifting and bending.
- Current First Aid Certificate including CPR
Qualification
Tertiary studies in Community Services (Certificate III/IV, Diploma, Degree) or equivalent
Previous employment or experience in the health, aged care, disability, and community services desirable.
Proficient computer skills, advanced knowledge in Microsoft Office Suite
