Disability & Youth Support is a disability support service specialising in ASD teens and young adults. We provide NDIS participants with a range of support options, including support coordination and Psychosocial recovering coaching. We Strive to empower participants to take charge of their lives, emphasizing not only their service needs but also helping them establish and sustain a robust network of both formal and informal support systems. This entails collaborating closely with participants to grasp their funding, identify their service expectations, and customize their desired service structure. Additionally, Coordination of Supports encompasses mentoring participants and actively aiding them in enhancing their capacity and resilience within their support networks.

The Role
As a NDIS Support Coordinator you will work with participants to search for, coordinate and implement all supports in their plan, including informal, mainstream, community and funded services and resources, whilst you actively support participants to build on their skills and work with them to increase their ability to access their local community and achieve greater independence.
You will be responsible for:
- Coordinating and connecting required supports for participants, with providers that will best support their need
- Ensuring participants service agreements, service bookings and NDIA reporting requirements are all managed effective and completed on time
- Ensuring participants understand their responsibilities under service agreements
- Researching and sourcing specialised services to meet the participants needs, such as Occupational Therapist, Speech Pathologist, Physiotherapist, Assistive Technology etc
- Preparing and monitoring the progress of participants goals and evaluating effectiveness of their supports
- Stay current with NDIS guidelines and regulations, attending relevant training and workshops as needed.
- Build positive relationships with clients, their families, support workers, and external service providers.
- Collaborate with our team to provide excellent support and services to our clients.
- Assist clients in understanding their NDIS plans and the services available to them.
To be successful in this role you will be a highly motivated and energetic candidate with strong verbal and written communication skills having worked in a similar role. You will be passionate and thrive on supporting participants with implementing their NDIS Plan successfully with exceptional outcomes for the participant and their families.
Required Experience, Skills and Qualifications:
- Minimum 12 months experience as a Support Coordinator preferred
- Relevant Tertiary qualifications including - Diploma in Community Services; or Certificate III or IV in Disability or equivalent; or significant demonstrated experience and skills in community services delivery
- A preferred candidate would have higher qualifications of a Bachelor or Masters qualifications and relevant experience to do Specialist Support Coordination.
- Lived experience is valued experience
- Successful experience in coordinating responsive, individualised support to participants with complex needs and their families
- A sound working knowledge of relevant legislation, NDIS procedural guidelines, NDIS price guide and the full range of community services that may be required by participants
- Advanced case notes and report writing experience
- Confident IT system skills, including cloud based software
- Ability to work autonomously
- Excellent communication skills
- Ability to assess risk and apply mitigation strategies
- Excellent administration skills and the ability to interpret information guides, processes, and policy documents
- Excellent time management and demonstrated ability to prioritise and self-motivate
- Advanced stakeholder relationship building, conflict resolution, and negotiation skills
- Solutions focused (a problem solving expert!)
- Current Queensland Drivers Licence
- A valid Blue Card and Yellow Card/NDIS National Screening Check
- First Aid / CPR
- Police Check
- Rights to Work in Australia
