Anala is seeking a Support Coordinator to join the team.

The role
The Support Coordinator is responsible for supporting participants, their family and carers in identifying and connecting with appropriate providers of supports with a view to overcoming barriers, developing skills and achieving the objectives of their NDIS Plan.
Duties
- Provide Support Coordination to assist the participant to implement their NDIS plan by maximizing choice and control and engaging high quality service providers.
- Be a key point of contact for the participant for the NDIA and all services and support to ensure the full implementation of the participants NDIS plan.
- Establish and maintain clear lines of communication and consultation between participants, their guardians, families, and other service providers, particularly in relation to Participant individual planning and related activities.
- Ensure participant goals and level of attainment is monitored, adjusted and recorded.
- Select preferred options or providers based on the chosen selection criteria.
- Arrange any assessments required to determine the nature and type of funding required.
- Monitor and adjust participants budgets to ensure NDIS plan outcomes are optimised.

Qualifications/License/checks
- Minimum Cert IV Qualification in Community Services Sector
- Apply for NDIS Worker Screening Check
- Working with Children Check
- Current Driver’s Licence and Vehicle
- NDIS Worker Orientation Module and New Worker - Induction Module
