SalarySummerland Support Services is currently seeking experienced and well organised Support Coordinators to join our team on a long-term basis, delivering exceptional support tailored to the diverse needs of our participants.
We have both full-time and part-time positions available.
Responsibilities include:
- Providing support coordination and/or psycho-social recovery coaching to NDIS participants with varying support requirements.
- Employing a person centred approach to enhance the capacity, choice, and control of participants.
- Collaborating with clients to develop and implement support plans, while monitoring progress and outcomes.
- Identifying and addressing complex challenges, barriers, and risks participants encounter, in conjunction with stakeholders.
- Demonstrating a solution focused mindset, encouraging innovative thinking.
- Contributing to organisational development and community networking efforts.
- Adapting to evolving participant circumstances.
- Cultivating a welcoming, inclusive, and respectful support network for individuals from diverse backgrounds.
Qualifications and experience required:
- A minimum of 2 years' experience in NDIS support coordination or a similar role.
- Qualifications in community services, disability, or related health disciplines, with a minimum of Cert 4.
- Thorough knowledge of NDIS operational guidelines.
- Possession of a current driver’s license and a comprehensively insured, safe vehicle.
- NDIS worker screen clearance and working with children check.
- Certificate of completion for the NDIS worker modules.
- First Aid and CPR certifications.
- Excellent interpersonal skills to establish productive relationships with various stakeholders.
- Permanent Australian working rights and current residency.
- Proficiency in computer literacy and the Microsoft suite.
- Ability to work independently and proactively.
