The OpportunityWe are currently seeking an experienced Level 2 Support Coordinator.
Responsibilities include:
- Support participants to build their plan including identifying all diagnosed treatment resistant disabilities, how this impacts their functional domains, their goals, the support needed to build their capacity to achieve their goals, connect with support services, establish and maintain supports.
- Liaising with NDIS providers, community, mainstream and other government agencies.
- Building effective relationships with participant’s and their families, nominees and informal supports while maintaining appropriate and professional boundaries.
- Coach participant’s how to use NDIS and other NDIS provider systems and processes.
- Refine and evaluate what is working for Bayley House participant’s and what isn’t working in their plan by tracking progress towards achieving goals with the participant and then coaching them on their options.
- Build a participant and/or nominee’s confidence to understand and implement their NDIS plan funds to access a mix of services and supports that increase capacity to maintain relationships, manage service delivery tasks, live more independently, create a meaningful life and be included in their community in accordance with the participants choice and control.
- Obtain reports from service providers, incident reports, risk assessments , allied health outcomes reports, functional assessments, GP assessments and statements from other service providers as evidence when assessing what is reasonable and necessary support to build a participant's capacity to meet their goals.
- Support the participant to develop service agreements with service providers.
- Manage allocated plan funding and outcomes to ensure plan services remain within budget.
- Coordinate and manage challenges within the participant’s care team of supports to ensure consistent delivery of services across the duration of the NDIS plan.
- Complete compliance reports of risk assessment, safety plans and support plans to maintain safeguard requirements.
- NDIS awareness in relation to choice and control and dignity of risk.
- Attend internal Bayley House team meetings and trainings as necessary.
About You
To be considered you will have significant experience working within the disability sector, including a comprehensive knowledge of the Disability Act, NDIS and other relevant legislation and standards. Two years minimum experience as a Support Coordinator.
You will also demonstrate:
- A comprehensive knowledge of the NDIS sector including up to date awareness of changes, planning and assessment processes.
- Excellent interpersonal skills with the ability to collaborate effectively with a broad range of internal and external stakeholders.
- Ability to network and develop and maintain relationships with Service Providers and advocacy groups.
- Have a Cert IV in Disability Services or equivalent in human services, with experience as a Support Coordinator
- Demonstrated ability to identify support needs and/or service gaps, plan for future needs, and explore innovative, cost-effective strategies to meet these needs.
- Ability to maintain appropriate records in a timely manner.
- Ability to collate and extract reporting data and meet reporting requirements.
- Current Drivers licence.
- Current NDIS Workers Screening Check
- Current understanding of working within an 80% billable workday model of employment.
