The support we provide is individualised, self-directed and integrated.
Continuity Care has an exciting opportunity for an experienced Support Coordinator looking for part-time role. This is a maternity leave cover role with the potential for extension as our service grows. This role is suitable for those looking to work 3-4 days per week or during school hours.
The Role:
- Work with NDIS participants, families, and other stakeholders to implement, monitor and review NDIS plans
- Support implementation of all supports in the plan, including informal, mainstream and community, as well as funded supports
- Connecting participants with both formal and informal supports to improve their quality of life
- Provide person-centred support to participants to achieve their goals
- Identify and coordinate appropriate service providers and supports for participants, including coordinating the implementation of service agreements
- Provide ongoing support and guidance to participants and their support network to ensure that goals are achieved and services are delivered in accordance with the participant's plan.
- Maintain accurate and up to date progress notes and participate in review and reporting processes with the NDIA and Local Area Coordinators.
Skills and Experience:
- Demonstrated experience in either support coordination or case management role.
- Effective time management, organisational skills and ability to prioritise tasks.
- An in-depth knowledge of the National Disability Insurance Scheme, including pricing arrangements, operational guidelines and NDIA reporting requirements.
- An ability to work effectively with people living with disability and/or mental health conditions to produce positive, person-led outcomes.
- Excellent verbal and written communication skills.
- Integrity, respect, empathy, and passion to make a difference.
Mandatory Requirements:
- Minimum 12 months previous experience in support coordination or case management is essential
- Cert IV or above in Disability, Mental Health or similar field is highly desired.

- Working with Children Check (Blue Card)
- Driver's Licence
