Support Coordinator - North / Central Metro

JP Trusted Advisor is on the search for a Support Coordinator to be part of our passionate team across the Central North Metro area. 

Duties

As a Support Coordinator you will be expected to assist participants funded under the NDIS to understand and use their NDIS plan to pursue their goals, connect with NDIS providers, community, mainstream and other government services. While building their individual capacity and skills to coordinate and manage their day to day supports and engagement in their community.

• Provide high quality customer service
• Actively listen to and respond appropriately to client feedback
• Be understanding of the customers’ needs
• Use a friendly and kind nature when interacting with clients
• Ability to network and build relationships with a range of external stakeholders
• Computer literate
• Effective report writing

Knowledge

• Understanding of the NDIS
• Understanding of the challenges faced by people with disabilities
• Good understanding of disability support services available in WA

Education, Experience and Qualifications

• Diploma in Community Services, Mental Health or other relevant qualification
• Minimum 2 years experience working as a Support Coordinator preferable otherwise; 
• Minimum 2 years Disability or Mental Health experience or lived experience

Essential Requirements

Will have or be prepared to obtain the following: 

• Current Driver's License
• Current NDIS Workers Screening Check
• Current Working with Children’s Check
• Satisfactory Completion of the NDIS Worker Orientation Module
• COVID-19 Vaccination
