In the combined role of Support Coordinator and Recovery Coach, you will play a pivotal role in identifying the needs, goals, and aspirations of our clients. You will be crucial in crafting a tailored support plan aimed at helping clients meet their desired outcomes. Your responsibilities will encompass providing continuous support coordination, information and guidance to our clients. As a recovery coach, your focus will be on aiding clients in managing their mental health and pursuing personal recovery objectives.
Key Responsibilities:
- Facilitate support coordination for clients, aiding them in navigating the NDIS framework and securing appropriate services
- An understanding of billable hours in relation to KPI targets
- Basic accounting and budgeting skills
- Collaborate with clients, their families, and other service providers to create and execute a personalized support plan
- Offer sustained support and capacity enhancement to clients
- Support clients in building social connections and participating in community activities
- Deliver recovery coaching to clients, concentrating on personal recovery aims, mental health management, and the development of coping mechanisms
- Collaborate with various service providers to secure optimal outcomes for clients
Qualifications:
- A minimum of 1 year of experience in support coordination and recovery coaching
- Certificate IV or Diploma in community services or relevant qualification. Social Work or other relevant allied health degree is highly sought after.
- Experience in justice interface, complex mental health, alcohol and other drugs.
- Proven knowledge and comprehension of NDIS guidelines and protocols
- Prior experience in assisting individuals with disabilities and/or mental health challenges
- Outstanding communication and interpersonal abilities
- Capability to operate both independently and as a team member
- Possession of a current Driver's License and access to a dependable vehicle
Benefits:
A competitive hourly rate from $55 to $100 dependent on the service and experience.
