An enthusiastic, self-motivated person to join our small team who provide Support Coordination (SC) to people over the age of 18 living in South Australia.

What you’ll be doing
Working with NDIS participants to support them to:
- Understand their NDIS plan and how they can use it
- Work out their needs and what supports will assist them
- Present options and choices available to them
- Link and coordinate with chosen services
- Communicate demonstrated needs to assist moving forward
What we need from you
- Integrity, learning & problem solving
- Evidence of how you’ve supported people to work towards their goals
- An understanding of how you communicate & can adjust your approach to make sure people get what they need
- A commitment to build peoples capacity
- Flexibility and commitment
Must haves
- Experience in the disability field
- Tertiary qualifications or equivalent training (Certificate) and/ or experience working in the field
- Australian Driver's license and vehicle
- NDIS Worker Screening check (or willing to obtain)
- DHS Aged Care screening (or willing to obtain)
- NDIS Worker Orientation Module (or willing to obtain)
- Proof of Vaccination Status
- A drive to come to work and assist people to work towards what is important to them
